A free decision tool that walks through the recording criteria in 29 CFR 1904 and tells you whether an injury or illness likely belongs on your OSHA 300 log, and in which column.
It asks three questions in order: is the case work-related (29 CFR 1904.5), is it a new case and not a recurrence of one already recorded (1904.6), and what was the most serious result (1904.7). A death goes in column G, days away from work in column H, and job transfer or restriction in column I. Medical treatment beyond first aid, loss of consciousness, or a significant injury or illness diagnosed by a physician goes in column J. A case where the only treatment is on the first-aid list in 29 CFR 1904.7(b)(5)(ii) is not recordable. Special rules apply to needlesticks, hearing loss, tuberculosis and musculoskeletal disorders. How to use it
- Say whether the injury or illness is work-related.
- Say whether it is a new case or a recurrence of a case you already recorded.
- Pick the single most serious outcome that applies.
- Read the result and the 300-log column. Open the first-aid list on the page to check whether the treatment given counts as first aid.
